1. CALL TO ORDER AND APPOINTMENT OF ALTERNATES
R. Muckle called the meeting to order at 6:30 p.m. I. Plis was seated for W. Thomas and J.
Wainscott seated for the vacancy.
2. APPROVAL of the May 27, 2021 MEETING MINUTES
Motion: Motion made by G. Renegar, second by J. Wainscott to approve the May 27, 2021
meeting minutes.
Vote: 6-0
3. APPLICATION REVIEW
C-21-04: 198 Shore Road: Stream Channel Stabilization — John Lombardi, Applicant &
Owner; Joseph Theroux, Agent.
Joseph Theroux and John Lombardi were available for the application. J. Theroux reviewed his
written responses to staff application review comments dated May 26, 2021. I. Plis questioned
how the rip-rap will be placed by the machinery. J. Theroux replied the excavator would deposit
the stone along the bank and push it into place, or in some locations it would be placed by hand.
Motion: Motion made by J. Wainscott, second by I. Plis to have staff prepare a draft
permit with conditions for review at the next meeting.
Vote: 6-0
4, NEW APPLICATIONS
C-21-6: 394 Willetts Avenue Ext. & 61A Myrock Avenue Install Private Gravity Fed
Sewer Main — Waterford Woods, LLC, Owner & Applicant, Yantic River Consultants, LLC;
Agent.
Brandon Handfield, PE reviewed the proposed plan. The proposed sewer extends across a
portion of 61A Myrock Avenue, west of the Eversource building, and through procured
easement rights over 61 Myrock Avenue. The approved site plan included a 1,500 ft. force
sewer main up to Willets Avenue and the proposed plan is to connect to the municipal sewer by
gravity. He noted the wetlands have been delineated and a wetland report will be submitted to
the Commission. The proposed activity includes a 20 foot wide construction corridor with
excavation between 3 to 3 % feet deep. Approximately 4,400 square feet of wetland area will be
affected. The pipe will be installed and the wetland material will be restored. The proposed
manholes are to be placed outside of the wetland. Within the upland review area, the sewer
installation will be 10 feet to 25 feet away from the wetland edge.

The Commission accepted the application for review.
5. VIOLATIONS
54 Rope Ferry Road — Status of Stream Restoration:
Staff updated the Commission the work is on schedule.
