This year we will discontinue offering Nurse Managed Clinics, Blood pressure and Foot clinics due to a return to our core business of Home Health. The budget for Yale New Haven Health- Health at Home- Southeast (YNHH-HAH- Southeast) service will include the home health skilled visit charge of $180 per nursing visit. YNHH-HAH Southeast is committed to providing healthcare services to the town at an appropriate level. The agency is a state-licensed, Medicare-certified, home care organization. YNHH- HAH Southeast underwent a reporting structure change. YNHH-HAH- Southeast reports to Yale New Haven Health System rather than Lawrence and Memorial Corporation. Indigent Care/Health Promotion Home visits to residents at risk for handicapping conditions or hospitalizations are made under consultation with the family physician. A plan is established to assist the resident to remain safely at home, Home healthcare services including nursing, therapies, home health aide and social workers are provided under a plan of care when the resident is uninsured or underinsured and meets the financial criteria for free or subsidized care. There is a continued need for assistance with care for those who are struggling to stay independent at home. The need for free care has reduced in the past few years due to the greater use of CT expanded Medicaid. At this time it is unknown whether the expansion will continue. For the future my cell phone is 203-313-0026. You may always contact me on that number as that is my personal cell. 1. Inthe Wellness programs we see approx. 52 patients per month. Most of these are people who come back month after month. Over the course of the year there were 621 visits to the clinics. In the indigent care program we see between 6-10 patients per month. Usually these are the same people as we maintain them outside of the hospital with monthly visits to help them remain in their homes. 2. Sunsetting these programs will allow the VNA to focus our time, talent and resources on our core business of caring for patients in their homes. The Board of Finance held the regular board meeting at the scheduled time (7:00pm) and finished extraordinarily early (7:31pm). The Budget Hearing meeting began early and the Board did not have after-hours contact information for you. Had we known you would have access to your work email, we would have resorted to that means of communication. I apologize for your commute to Town Hall and no one being present to meet to you after soliciting you to attend the meeting. All of that being said, the Board of Finance has approved your budget of $21,600. The Board posed two (2) questions for you and if you can kindly respond via email, it will be appreciated. 1. What is the number of Waterford residents that the VNA has serviced in the past year? 2. What effects has the change of Yale New Haven cutting funding/programs had on the VNA?
