PHASE I - CONSTRUCTION STAGING AREA, ROCK PROCESSING AREA, DRIVEWAY AND ASSOCIATED UTILITIES, STORMWATER
BASINS 100, 110, 120.
1.
PRIOR TO COMMENCEMENT OF WORK A PRE-CONSTRUCTION MEETING SHALL BE HELD WITH TOWN STAFF AND
REPRESENTATIVES OF THE CONTRACTOR AND OWNER. AT THIS MEETING, ONE PERSON WILL BE PLACED IN CHARGE OF
SEDIMENT AND EROSION CONTROL FOR THE ENTIRE SITE.
2.
CONTRACTOR TO STAKE OUT LIMIT OF DISTURBANCE AND VEGETATION TO BE RETAINED.
3.
INSTALL SEDIMENT AND EROSION CONTROLS ALONG THE PERIMETER, AND INSTALL STABILIZED CONSTRUCTION ENTRANCES,
AS SHOWN ON THE PLANS.
4.
REMOVE STUMPS AND STRIP TOPSOIL IN CONSTRUCTION STAGING AREA AND ALONG CONSTRUCTION ACCESS ROADS. SOIL
IS TO BE STOCKPILED AND SURROUNDED BY SEDIMENT FILTER FENCE AND HAY BALES.
5.
INSTALL DIVERSION BERMS AND SEDIMENT TRAPS PER THE SEDIMENT AND EROSION CONTROL PLAN FOR THIS PHASE.
TEMPORARY SEDIMENT TRAPS AND DIVERSION BERMS AND SWALES ARE TO BE CONSTRUCTED PRIOR TO EACH PHASE OF
GRADING AND MODIFIED AS NECESSARY TO FUNCTION. CLEAN TEMPORARY SEDIMENT TRAPS PER THE MAINTENANCE
INTERVALS OR SOONER, IF NECESSARY.
6.
INSTALL OUTLET SCOUR HOLES AND SPLASH PADS. BEGIN PIPE INSTALLATION UPGRADIENT FROM OUTFALLS.
7.
INITIATE MASS EARTHWORK OPERATIONS FOR ROCK PROCESSING AREA, AND CONSTRUCTION OF ROAD.
8.
SLOPES ARE TO BE ESTABLISHED AS SOON AS PRACTICAL BEFORE UTILITY INSTALLATION. STABILIZE ALL SLOPES IMMEDIATELY
AFTER THEIR ESTABLISHMENT.
9.
INSTALL ROAD UTILITIES AND CURBS UP TO DETENTION BASIN 130. PLACE PERMANENT SEEDING AT AREAS THAT WILL NO
LONGER BE DISTURBED DUE TO CONSTRUCTION.
10. CLEAN STORM DRAINAGE SYSTEM OF ALL SEDIMENT AND DEBRIS. FINISH GRADE BOTTOM OF STORMWATER BASINS AND
OUTLET CONTROLS CREATING FOREBAYS.
11. COMPLETE PLANTING AND PERMANENT STABILIZATION OF STORMWATER MANAGEMENT BASINS AND OUTLETS.
12. THE SEDIMENT AND EROSION CONTROL PLAN SHALL BE MODIFIED BY THE CONTRACTOR AS NECESSITATED BY CHANGING
SITE CONDITIONS OR AT THE DIRECTION OF THE ENGINEER AND/OR DESIGNATED TOWN REPRESENTATIVE.
13. ALL DEWATERING WASTE WATERS SHALL BE DISCHARGED IN A MANNER WHICH MINIMIZES THE DISCOLORATION OF THE
RECEIVING WATERS
14. THE SITE SHOULD BE KEPT CLEAN OF LOOSE DEBRIS, LITTER, AND BUILDING MATERIALS SUCH THAT NONE OF THE ABOVE
ENTER WATERS OR WETLANDS.
PHASE II - BUILDING CONSTRUCTION, REMAINING DRIVEWAY, UTILITIES, AND REMAINING STORMWATER BASINS.
1.
INSTALL TEMPORARY SEDIMENT TRAP(S) FOR THIS PHASE. DISTURBED WORK AREAS TO BE LIMITED TO 5 ACRES OR LESS
WITH RUNOFF CONTROLLED TO A TEMPORARY SEDIMENT TRAP.
2.
CONSTRUCT TEMPORARY ACCESS ROADS TO WORK AREAS.
3.
REMOVE STUMPS AND STRIP TOPSOIL IN WORK AREAS. STOCKPILE AND SURROUND BY SEDIMENT FILTER FENCE AND HAY
BALES.
4.
BEGIN MASS EXCAVATION OF BASINS AND FORMATION OF EMBANKMENTS. CONSTRUCT BASINS, OUTLET STRUCTURES AND
STABILIZE AS SOON AS POSSIBLE.
5.
INITIATE MASS EARTHWORK OPERATIONS IN WORK AREAS TO APPROACH ROUGH SUBGRADE.
6.
BEGIN EXCAVATION FOR BUILDING FOUNDATIONS. BEGIN CONCRETE WORK FOR FOOTINGS AND FOUNDATION WALLS IN
COUNTERCLOCKWISE SEQUENCE FROM SOUTH TO NORTH OR AS DESIGNATED BY OWNER.
7.
AS FOUNDATION WALLS ARE UP THE BUILDING FOOTPRINTS ARE PERMANENTLY STABILIZED, BEGIN INSTALLING
UNDERGROUND UTILITIES FOR EACH BUILDING.
8.
INSTALL REMAINING UTILITIES UNDER AND AROUND PARKING AREAS. PLACE BINDER COURSE OF PAVEMENT AS BASE
COURSE IS BROUGHT UP TO GRADE.
9.
COMPLETE ALL SITE IMPROVEMENTS.
10. PLACE PERMANENT SEEDING AT AREAS DISTURBED DUE TO CONSTRUCTION AND INSTALL PLANTINGS.
11. CLEAN STORM DRAINAGE SYSTEM OF ALL SEDIMENT AND DEBRIS. FINISH GRADE BOTTOM OF STORMWATER BASINS AND
OUTLET CONTROLS CREATING FOREBAYS.
12. COMPLETE ALL PLANTINGS AND PERMANENT STABILIZATION OF STORMWATER MANAGEMENT BASINS AND OUTLETS.

STORMWATER MANAGEMENT OPERATION AND MAINTENANCE PLAN
1. RECOMMENDED FREQUENCY OF SERVICE:
AS FURTHER DEFINED BELOW, ALL STORMWATER COMPONENTS SHOULD BE CHECKED ON A PERIODIC BASIS AND KEPT IN FULL WORKING ORDER.
ULTIMATELY, THE REQUIRED FREQUENCY OF INSPECTION AND SERVICE WILL DEPEND ON RUNOFF QUANTITIES, POLLUTANT LOADING, AND CLOGGING
DUE TO DEBRIS. AT A MINIMUM, WE RECOMMEND THAT ALL STORMWATER COMPONENTS BE INSPECTED AND SERVICED TWICE PER YEAR, ONCE
BEFORE WINTER BEGINS AND ONCE DURING SPRING CLEANUP.
2. QUALIFIED INSPECTOR:
THE INSPECTIONS MUST BE COMPLETED BY AN INDIVIDUAL EXPERIENCED IN THE CONSTRUCTION AND MAINTENANCE OF STORMWATER DRAINAGE
SYSTEMS. ONCE EVERY FIVE YEARS THE INSPECTIONS MUST BE COMPLETED BY A PROFESSIONAL ENGINEER.
3. SERVICE PROCEDURES:
1. CATCH BASINS & DRAINAGE INLETS:
a. CATCH BASINS AND DRAINAGE INLETS SHALL BE COMPLETELY CLEANED OF ACCUMULATED DEBRIS AND SEDIMENTS AT THE COMPLETION OF
CONSTRUCTION.
b. FOR THE FIRST YEAR, CATCH BASINS AND DRAINAGE INLETS SHALL BE INSPECTED ON A QUARTERLY BASIS.
c. ANY ACCUMULATED DEBRIS WITHIN THE CATCH BASINS/INLETS SHALL BE REMOVED AND ANY REPAIRS AS REQUIRED.
d.FROM THE SECOND YEAR ONWARD, VISUAL INSPECTIONS SHALL OCCUR TWICE PER YEAR, ONCE IN THE SPRING AND ONCE IN THE FALL,
AFTER FALL CLEANUP OF LEAVES HAS OCCURRED.
e. ACCUMULATED DEBRIS WITHIN THE CATCH BASINS/INLETS SHALL BE REMOVED AND REPAIRS MADE AS REQUIRED.
f. ACCUMULATED SEDIMENTS SHALL BE REMOVED AT WHICH TIME THEY ARE WITHIN 12 INCHES OF THE INVERT OF THE OUTLET PIPE.
g. ANY ADDITIONAL MAINTENANCE REQUIRED PER THE MANUFACTURER'S SPECIFICATIONS SHALL ALSO BE COMPLETED.
2. STORM DRAINAGE PIPING AND MANHOLES:
a. ALL STORM DRAINAGE PIPING SHALL BE COMPLETELY FLUSHED OF DEBRIS AND ACCUMULATED SEDIMENT AT THE COMPLETION OF
CONSTRUCTION.
b. MANHOLES SHALL BE INSPECTED AND REPAIRED ON AN ANNUAL BASIS.
c. UNLESS SYSTEM PERFORMANCE INDICATES DEGRADATION OF PIPING, COMPREHENSIVE VIDEO INSPECTION OF STORM DRAINAGE PIPING
SHALL OCCUR ONCE EVERY TEN YEARS.
d. ANY ADDITIONAL MAINTENANCE REQUIRED PER THE MANUFACTURER'S SPECIFICATIONS SHALL ALSO BE COMPLETED.
3. STORMWATER BASINS:
a. MOWING: THE UPPER STAGE, SIDE SLOPES, AND EMBANKMENT OF STORMWATER POND MUST BE MOWED AT LEAST ONCE PER YEAR TO
DISCOURAGE WOODY GROWTH AND CONTROL WEEDS.
b. INSPECTIONS: BASIN SHOULD BE INSPECTED TWICE PER YEAR(SPRING AND FALL) TO ENSURE THAT THE STRUCTURE
OPERATES IN THE MANNER ORIGINALLY INTENDED. WHEN POSSIBLE, INSPECTIONS SHOULD BE CONDUCTED DURING WET
WEATHER TO DETERMINE IF THE BASIN IS MEETING THE TARGETED DETENTION TIMES PER APPROVED DESIGN. IN
PARTICULAR, THE OUTLET CONTROL DEVICE SHOULD BE REGULARLY INSPECTED FOR EVIDENCE OF CLOGGING OR,
CONVERSELY, FOR TOO RAPID A RELEASE, AND THE FLOW PATH SHOULD BE CHECKED FOR EROSION PROBLEMS. OTHER
PROBLEMS THAT SHOULD BE CHECKED FOR INCLUDE SUBSIDENCE, OUTLET WATER TURBIDITY, BANK/BED/OUTLET EROSION,
CRACKING, OR TREE GROWTH ON THE EMBANKMENT; THE ACCUMULATION OF SEDIMENT AROUND THE OUTLET; THE
ADEQUACY OF UPSTREAM/DOWNSTREAM CHANNEL EROSION CONTROL MEASURES; AND MODIFICATIONS TO THE BASIN OR ITS
CONTRIBUTING WATERSHED THAT MAY INFLUENCE BASIN PERFORMANCE. INSPECTIONS SHOULD BE CARRIED OUT WITH
DESIGN PLANS IN HAND.
c. DEBRIS AND LITTER REMOVAL: DEBRIS AND LITTER WILL ACCUMULATE NEAR THE OUTLET CONTROL DEVICEAND SHOULD BE
REMOVED DURING REGULAR INSPECTION AND/OR MOWING OPERATIONS. PARTICULAR ATTENTION SHOULD BE PAID TO
FLOATABLE DEBRIS THAT COULD EVENTUALLY CLOG THE CONTROL DEVICE OR RISER.
d. SEDIMENT REMOVAL: WHEN PROPERLY DESIGNED, DETENTION/WATER QUALITY BASINS WILL ACCUMULATE SEDIMENT OVER
TIME. HOWEVER, MOST OF THE SEDIMENT WILL BE TRAPPED IN THE WATER QUALITY UNITS AND CATCH BASIN SUMP UNITS
BEFORE REACHING THE BASIN. THE REMAINDER WILL ACCUMULATE IN THE STORMWATER POND. ACCUMULATED SEDIMENT
MUST BE REMOVED FROM THE BASIN EVERY 5 YEARS, AFTER ONE HALF (15"±) OF THE SEDIMENT STORAGE CAPACITY IN THE
FOREBAY HAS BEEN FILLED, AFTER 4 INCHES OF SEDIMENT HAS ACCUMULATED IN THE MAIN PORTION OF THE BASIN, OR
WHEN SIGNIFICANT ALGAL GROWTH IS OBSERVED. A PERMANENT MEASURING DEVICE SHALL BE INSTALLED IN THE MIDDLE OF
THE FOREBAY AND IN THE MAIN PORTION OF THE BASIN. THE MARKER SHALL DELINEATE INCHES UP FROM THE BOTTOM OF
THE BASIN SO THE DEPTH OF SEDIMENT CAN EASILY BE MEASURED. MORE FREQUENT SPOT CLEANOUTS MAY BE NEEDED
AROUND THE OUTLET CONTROL DEVICE OR THE SEDIMENT FOREBAY.
SEDIMENT REMOVAL OPERATIONS ARE RELATIVELY SIMPLE. FRONT-END LOADERS, BACKHOES, OR VACUUM TRUCKS CAN BE
USED TO REMOVE THE ACCUMULATED SEDIMENT FOLLOWED BY MANUAL REMOVAL OF SEDIMENT DEPOSITED AROUND THE
OUTLET CONTROL DEVICE. THE SEDIMENT SHALL BE DISPOSED OF IN AN APPROVED OFF-SITE LOCATION IN ACCORDANCE
WITH TOWN AND STATE REQUIREMENTS. THE DISTURBED AREA SHOULD BE IMMEDIATELY SEEDED WITH APPROPRIATE GRASS
SEED AND MULCHED WITH HAY AFTER REMOVAL OPERATIONS ARE COMPLETED TO PREVENT THE OUTLET CONTROL DEVICE
FROM CLOGGING.
4. STORMWATER CONTROL STRUCTURES:
a. ALL CONTROL STRUCTURES (ORIFICE, WEIR, ETC.) SHALL BE COMPLETELY CLEANED OF ACCUMULATED DEBRIS AND SEDIMENTS AT THE
COMPLETION OF CONSTRUCTION. ANY REPAIRS SHALL BE PERFORMED.
b. FOR THE FIRST YEAR, CONTROL STRUCTURES (ORIFICE, WEIR, ETC.) SHALL BE INSPECTED ON A QUARTERLY BASIS.
c. ANY ACCUMULATED DEBRIS SHALL BE REMOVED AND ANY REPAIRS MADE TO THE CONTROL STRUCTURES (ORIFICE, WEIR, ETC.) AS
REQUIRED.
d.FROM THE SECOND YEAR ONWARD, VISUAL INSPECTIONS SHALL OCCUR TWICE PER YEAR, ONCE IN THE SPRING AND ONCE IN THE FALL,
AFTER FALL CLEANUP OF LEAVES HAS OCCURRED.

5. WATER QUALITY UNITS:
a. WATER QUALITY UNITS SHALL BE COMPLETELY CLEANED OF ACCUMULATED DEBRIS AND SEDIMENTS AT THE COMPLETION OF
CONSTRUCTION.
b. FOR THE FIRST YEAR, THE HYDRODYNAMIC SEPARATOR SHALL BE INSPECTED ON A QUARTERLY BASIS.
c. ANY ACCUMULATED DEBRIS WITHIN THE HYDRODYNAMIC SEPARATOR SHALL BE REMOVED AND ANY REPAIRS MADE TO THE UNIT AS
REQUIRED.
d.FROM THE SECOND YEAR ONWARD, VISUAL INSPECTION SHALL OCCUR TWICE PER YEAR, ONCE IN THE SPRING AND ONCE IN THE FALL, AFTER
FALL CLEANUP OF LEAVES HAS OCCURRED.
e. ACCUMULATED DEBRIS WITHIN THE UNIT SHALL BE REMOVED AND REPAIRS MADE AS REQUIRED.
f. ACCUMULATED SEDIMENTS SHALL BE REMOVED AT WHICH TIME THEY ARE WITHIN 12 INCHES OF THE INVERT OF THE OUTLET PIPE.
g. ALL INLETS, OUTLETS AND COMPONENTS OF THE UNIT SHALL BE INSPECTED AND CLEARED OF DEBRIS. ANY REPAIRS SHALL BE PERFORMED.
h. ANY ADDITIONAL MAINTENANCE REQUIRED PER THE MANUFACTURER'S SPECIFICATIONS SHALL ALSO BE COMPLETED.
6. DRAINAGE OUTFALLS/SPLASH PADS/SCOUR HOLES:
a. ALL OUTFALLS SHALL BE COMPLETELY CLEANED OF ACCUMULATED DEBRIS AND SEDIMENTS AT THE COMPLETION OF CONSTRUCTION. ANY
REPAIRS TO OUTLET PROTECTION MATERIAL (RIP RAP) SHALL BE PERFORMED.
b. FOR THE FIRST YEAR, OUTFALLS SHALL BE INSPECTED ON A QUARTERLY BASIS.
c. ANY ACCUMULATED DEBRIS SHALL BE REMOVED AND ANY REPAIRS MADE TO THE OUTFALLS AS REQUIRED.
d.FROM THE SECOND YEAR ONWARD, VISUAL INSPECTIONS SHALL OCCUR TWICE PER YEAR, ONCE IN THE SPRING AND ONCE IN THE FALL,
AFTER FALL CLEANUP OF LEAVES HAS OCCURRED.
e. ACCUMULATED DEBRIS SHALL BE REMOVED AND REPAIRS MADE AS REQUIRED.
f. ANY EROSION SHALL BE PROMPTLY REPAIRED AND THE CAUSE OF THE EROSION SHALL BE IDENTIFIED AND CORRECTED.
g. ANY ADDITIONAL MAINTENANCE REQUIRED PER THE MANUFACTURER'S SPECIFICATIONS SHALL ALSO BE COMPLETED.
7. WATER QUALITY BASINS:
a. WATER QUALITY BASINS SHALL BE CLEANED OF DEBRIS AND SEDIMENTS UPON THE COMPLETION OF CONSTRUCTION. THE BASIN SHALL BE
VISUALLY INSPECTED ON A MONTHLY BASIS FOR THE FIRST 6 MONTHS. ANY EROSION SHALL BE PROMPTLY REPAIRED AND THE CAUSE OF THE
PROBLEM SHALL BE IDENTIFIED AND CORRECTED. MONTHLY INSPECTIONS SHALL CONTINUE UNTIL SUCCESSFUL OPERATION OF THE SYSTEM
IS CONFIRMED.
b. THE BASIN SHALL NOT BE MOWED MORE THAN TWICE DURING THE GROWING SEASON, PREFERABLY ONLY IN LATE OCTOBER. MORE
FREQUENT MOWING WILL ELIMINATE NATIVE FORBS AND SEDGES FROM THE MEADOW COVER.
c. ALL DEAD PLANTS SHALL BE REPLACED AND ANY NECESSARY PRUNING OF VEGETATION SHALL BE COMPLETED.
d. THE SURFACE OF THESE STRUCTURES SHALL BE INSPECTED ON A QUARTERLY BASIS AFTER THE FIRST SIX MONTHS OF SUCCESSFUL
OPERATION AND AFTER HEAVY RUNOFF EVENTS (E.G. >3.0” IN A 24-HOUR PERIOD). ONE INSPECTION SHALL OCCUR IMMEDIATELY
FOLLOWING THE COMPLETION OF WINTER SANDING AND SUBSEQUENT SWEEPING OPERATIONS, AND ONE SHALL OCCUR JUST PRIOR TO
THE WINTER SEASON. ANY ACCUMULATED DEBRIS AND SEDIMENTS SHALL BE REMOVED.
8. UNDERGROUND DETENTION SYSTEMS:
a.UNDERGROUND DETENTION SYSTEMS SHALL BE INSPECTED QUARTERLY AND SEDIMENT SHALL BE REMOVED AS NEEDED TO ENSURE PROPER
FUNCTIONING OF STRUCTURES. AREAS OF DISTURBANCE THAT MAY BE AS A RESULT OF CLEANING SHALL BE SEEDED AND PLANTED IN
ACCORDANCE WITH THE ORIGINAL PLANTING PLAN. THESE STRUCTURES WILL BE MAINTAINED YEARLY, OR MORE FREQUENTLY AS REQUIRED.
WASTE MATERIAL WILL BE PROPERLY DISPOSED OF OFF-SITE.
b.ANY ADDITIONAL MAINTENANCE REQUIRED PER THE MANUFACTURER'S SPECIFICATIONS SHALL ALSO BE COMPLETED.
9. ROOF GUTTERS
a. REMOVE ACCUMULATED DEBRIS AND INSPECT FOR CLOGGING AND/OR DAMAGE AT LEAST ONCE A YEAR, TYPICALLY IN THE
FALL AFTER THE LEAVES HAVE FALLEN. ANY DAMAGE SHOULD BE REPAIRED AS REQUIRED.
10. PAVEMENT SWEEPING
a. THE PARKING AREA AND ROADWAY SHALL BE SWEPT ANNUALLY. SWEEPING SHOULD OCCUR IN THE SPRING AFTER WINTER
SANDING, BETWEEN APRIL 15 AND MAY 15. SALT ALTERNATIVES SHALL BE USED DURING WINTER MONTHS FOR DEICING.
4.DISPOSAL OF DEBRIS AND SEDIMENT:
ALL DEBRIS AND SEDIMENT REMOVED FROM THE STORMWATER STRUCTURES SHALL BE DISPOSED OF LEGALLY. THERE SHALL BE NO DUMPING OF SILT
OR DEBRIS INTO OR IN PROXIMITY TO ANY INLAND WETLANDS.
5. MAINTENANCE RECORDS:
THE OWNERS(S) MUST MAINTAIN ALL RECORDS (LOGS, INVOICES, REPORTS, DATA, ETC.) AND HAVE ALWAYS THEM READILY AVAILABLE FOR
INSPECTION.
