ESC Checklist - 1 - Town of Waterford, CT CHECKLIST FOR EROSION AND SEDIMENT CONTROL PLAN. Drafting Standards - All maps, plans and profiles shall conform with Class A-2 of the Code of Recommended Practice for standards of accuracy of maps prepared by the Connecticut Technical Council, Incorporated. All maps, plans and profiles shall be presented on good quality white prints and shall be not more than thirty-six (36) inches long or twenty-four (24) inches wide. All such prints shall have a one-half (1/2) inch border on three (3) sides and a two (2) inch border on the left side. If more than one sheet is submitted, they shall be bound. Basic Information. All sheets must contain the following basic information (if applicable): _____ 1. Name and address of proposed project _____ 2. Name and address of property owner and applicant _____ 3. Name, address and telephone number of person or firm preparing plan _____ 4. Date of plan/materials preparation and any revision dates _____ 5. Graphic scale and true north arrow on any graphic representations _____ 6. Assessor’s Map number and unique ID number(s) of the land parcel(s) _____ 7. Legend identifying the symbols used _____ 8. Property boundary lines Erosion and Sediment Control Plan – Site plans prepared for the Erosion and Sediment Control Plan shall be drawn in accordance with the drafting standards provided at the beginning of this checklist. Where the site must be divided into multiple sheets in order to accommodate scale, a plan key shall be provided. A. The Erosion & Sediment Control site plan shall be at sufficient scale and include: _____ 1. Site locus map at 1:24,000 scale showing the location of the proposed development and adjacent properties _____ 2. Existing and proposed topography (2 ft. contours with supplemental spot elevations as necessary) _____ 3. Soil types, bedrock outcrops, steep slopes _____ 4. Boundaries of inland wetlands, watercourses and water bodies _____ 5. Tidal wetland boundaries and coastal resource limits _____ 6. FEMA identified flood zones and flood hazard areas _____ 7. Existing vegetation and proposed clearing extent _____ 8. Drainage patterns, drainage areas and size, existing and proposed. _____ 9. Size and location of existing and proposed culverts, channels, swales and stormwater conveyance and treatment structures ESC Checklist - 2 - Town of Waterford, CT _____ 10. Existing and proposed buildings, roads, utilities, water supply and septic systems _____ 11. The proposed limits of cuts, fills, grading, and clearing _____ 12. Cross sections of all areas of cuts and fills showing existing and proposed surface elevations _____ 13. The location of, and design details for, all proposed soil erosion and sediment control measures and all stormwater control and treatment measures _____ 14. Location of vehicle tracking pads, temporary access roads, soil and material stockpiles, equipment storage and fueling areas _____ 15. A plan indicating the phasing and sequencing of clearing, grading, excavation and construction activities _____ 21. Location and description of temporary measures to minimize soil disturbance, divert and control run-off, and capture and control sediment, _____ 22. Location and type of temporary and permanent soil stabilization measures _____ 23. The sequence of application for all temporary and permanent site stabilization measures, _____ 24. Location of proposed vegetation /cover types with details of plants, seeding, mulch, fertilizer, and planting/seeding dates _____ 25. Location and description of all tree protection practices where applicable _____ 26. A Certification block entitled “Erosion and Sediment Control Plan Certified by Vote of the Waterford Planning & Zoning Commission (date) and a space for the signature of the Chairman or Secretary of the Commission _____ 27. For plans containing E&S or stormwater control measures which require an engineered design, the plans shall include the signature and seal of a professional engineer licensed in the State of Connecticut _____ 28. Any other information deemed necessary and appropriate by the applicant or requested by the Commission or its designated agent. Erosion and Sediment Control Plan Narrative – An erosion and sediment control plan narrative shall accompany the site plan. B. Calculations for the amount of earth materials to be removed from or brought to the site _____ 9. Calculations for sizing the selected E&S Control and Best Management Practices _____ 10. The phases of construction and development _____ 11. Measures incorporated into the plan to divert off-site run-off, control on-site run-off velocity and control erosion from concentrated flows _____ 12. Measures proposed to contain and control sediment and dust _____ 13. The schedule for grading and construction activities including: a. start and completion dates b. sequence of grading and construction activities c. sequence for installation and/or application of soil erosion and sediment control measures, including temporary stabilization, and stormwater run- off control measures d. sequence and specifications for final stabilization of the project site, including all final landscaping e. contingency or emergency plans in case of failed erosion and sediment control systems and installations f. proper disposal of cleared, excavated or stockpiled material. _____ 14. An inspection and maintenance program for all temporary soil erosion and sediment control measures (at a minimum in accordance with the 2002 CT E&S Guidelines) _____ 18. An inspection and maintenance program for all temporary stormwater control measures (at a minimum in accordance with the 2004 CT Stormwater Quality Manual) _____ 19. Identification of a designated on-site individual responsible for installation, monitoring and correction of sediment control plan requirements, and authorized to take corrective actions as required to ensure compliance with certified plans _____ 20.
